Each patient on TransMed Network™ has a unique Electronic Health Record (EHR) or medical record. This record is originated by a patient’s primary physician and once placed on he Network, is referred to as the Shared Patient Chart.
This Shared Patient Chart is composed of a mosaic of non-editable reports (consultations, therapeutic reports, imaging records, etc.) retrieved and gathered by the Network and arranged in chronological order. This record is then accessible on transmed.net by any credentialed Member-Provider. The patient must give permission to access to his/her EHR.
Once in the TransMed Network™ Data Center, the electronic health record becomes a centralized health record that is updated on a real time basis. Using proprietary software technology, identical synchronized copies of this record are placed in local databases of those Member-Providers registered and credentialed by the patient.
